Accidental tourist

How do I obtain a blank 'European Accident Statement' form in English? A contact at my current motor insurer says he has never heard of it in his five years with the company. The policy booklet from my previous insurer advised taking it with me when going to Europe, but despite repeated requests, would not or could not send me one. Contacting the EU bureaucracy also led nowhere.

From the AA or the RAC, and they are included in some of the foreign touring kits you can buy at ferry ports, etc.
