Can you recommend a comfortable, economical double cab pick-up?

I am having a few problems with my lower back due to the seating position in my 2008 Ford Ranger Thunder, which I'm very fond of. It is a hoot to drive, very reliable and looks good. If I were to change to another double cab pickup, what would you recommend? Seating comfort, ability to transport four passengers occasionally and fuel economy are a priority - I cover around 20,000 miles per year.

By far the best of the previous generation was the 2007 - 2012 Isuzu D-Max (called Rodeo in the UK) with either the 2.5 I-TEQ or 3.0 I-TEQ chain cam diesel engines. Lovely 5-speed transmission, nice power delivery and big squishy seats. Our 2.5s did 40mpg, but they were 2WD. 4WD does about 36mpg. It seems that the latest model Isuzu D-Max, now with a twin turbo 2.5 diesel, is the best as well. I had thought that the new Ford Ranger would be, but a few quality issues have arisen.
