Incorrect fuelling could be eliminated by changing the cross section design of the pump nozzle and fuel tank filler pipe. For petrol the nozzle and filler pipe could continue to be round, for diesel the filler pump nozzle and filler pipe should be square section. With the correct dimensions it's possible to ensure that only the petrol pump nozzle would fit the petrol filler and only the diesel nozzle would fit the diesel filler.

Sounds a great idea. It makes no sense to have either nozzle fit the wrong car. You would think this would have been resolved before now.

A diesel nozzle won't fit into a petrol filler neck (except very old models).

A petrol nozzle shouldn't fit into a diesel neck with requires a nozzle of sufficient girth to open a flap, but few manufacturers fit this.

The technology is out there.
