Has it definetly had a LEXIA diagnostic? Third party generic readers can sometimes be very poor at diagnosing.
I had a very similar problem to you, always on the motorway, when warm, car would do a shudder like fuel starvation. I keep the car regularly serviced and the fuel filter had only been done a few thousand miles before.
It turned out to be the diesel heater sensor. I wasn't getting an engine management light on the dash. A generic code reader didn't pick up a fault. But the Lexia equipment did. Reading the live data, it showed that the percieved temeperature was way off reference.
New heater sensor unit and it was perfect and has been ever since.
