Renewing photo license eye sight - TheHonestMotorist

I hope I'm posting this in the right place. I've got to renew the mugshot on my photo card and I've come to the bit about my vision. I wear glasses when driving due to being short sighted, but do I need to fill in the V1 form and send it with my application? I don't have any other vision issues.

Unfortunately, it appears that the various licence forms aren't available for general view online - that I could see anyway.

If you could post a link to the V1 form or the form you're filling in, or maybe someone else can find the form and help out.
