My Honda Civic automatic sometimes lurches forwards - how can I prevent this?

I have a Honda Civic Hybrid automatic. On two occasions the Honda has lurched forward from starting and I haven't had time to react before hitting something just ahead, on the last occasion a brick wall. The obvious assumption is that I must have stamped on the accelerator pedal rather than the brake, or possibly both. I find this implausible, however, and wonder whether there is any mechanical fault on the Honda that could explain it?

The extremely simple and obvious answer is to drive two footed, using your left foot for the footbrake. Then you can stop the car in the space of a couple of feet whatever happens.
