Trade Plates

I am hoping to apply for trade plates and, if necessary, an operating license. My question is which forms do I need, and where do I get them ?

From www.dvla.gov.uk. They are issued by local DVLA offices and, depending on what you want them for, they first send someone to inspect your trading books and your premises.
